Use this database to log problems that have already occurred during the project. Issues are active problems that need to be resolved — they are not the same as risks.

⚠️ Risk vs. Issue: A risk is a potential future problem (log it in the Risk Register). An issue is a problem happening right now (log it here).

How to use:

1. Click + New to log a new issue
2. Set the Severity (Critical · High · Medium · Low) immediately
3. Assign an Owner responsible for resolution
4. Update Status as the issue progresses → Open · In Progress · Resolved
5. Fill in Resolution Notes when closing — never delete resolved issues, they form your audit trail
